A fraud reporting form to be used by law firm staff when a fraud is discovered or suspected. The form prompts the staff member to provide all the information necessary for the firm’s risk and compliance team to be able to triage the incident quickly and take it forward, in accordance with the firm’s fraud response plan (FRP).
